Workplace Investigation Report
Summary
This template is a workplace investigation report for use by employers to document an internal investigation of employment-related claims. This template includes practical guidance, drafting notes, and alternate clauses. This template is intended for private employers. It is based on federal law and does not address all potential state law distinctions; thus, you should check any relevant state and local laws. The workplace investigation report should include a summary of the reason(s) for the investigation, the factual findings obtained during interviews, and an analysis. You should also label the Report with the following header: "DO NOT DISTRIBUTE, CONFIDENTIAL, ATTORNEY-CLIENT PRIVILEGED, ATTORNEY WORK PRODUCT." This template should only be distributed to a limited number of individuals at the company.
