Triennial Statement

This Triennial Statement (NY Professional Service Corporation) may be used to file the triennial statement of a New York professional service corporation. This template includes practical guidance, drafting notes, and alternate clauses. A New York professional service corporation must, at least once every three years on or before the date prescribed by the relevant licensing authority, furnish a statement to the licensing authority listing the name and residence address of each shareholder, director, and officer of the corporation. Further, the corporation must certify that each of those individuals (or, in the case of a design professional corporation, the specified individuals) are authorized by law in New York to practice a profession which such corporation is authorized to practice.
