Summary

This template is an entry of appearance for counsel pursuant to the Pennsylvania Rules of Civil Procedure. It is used to give notice to the court and other parties that you act on behalf of a party in the underlying proceeding. This template contains practical guidance and drafting notes. Pa. R. Civ. P. 1012(a) provides that a party may enter a written appearance that states an address at which pleadings and other legal papers may be served and a telephone number. Pa. R. Civ. P. 1012(a); Pa. R. Civ. P. 440. Pa. R. Civ. P. 1012(f)(1) provides forms for the praecipe for entry of appearance, for the praecipe for withdrawal of appearance, and the substitution of counsel.
