Summary

This template is a Personal Information Access Request that could be used by a Canadian organization to allow individuals to request access to their personal information. Note that individuals cannot be required to use the specific form and may make a request by submitting a letter with substantially similar information. This template includes practical guidance and drafting notes. An organization may follow-up with the individual to obtain more information relating to the request in order to clarify and narrow the scope of the request.
