Summary

This Official Form 410S1 is a supplement to a proof of claim to be filed in an individual debtor’s Chapter 13 bankruptcy case where the claim is secured by a security interest in the debtor’s principal residence. The standard proof of claim form used in bankruptcy courts is Official Form B410 which is a three-page form. Where applicable, this form is a supplement to a proof of claim that must be attached to the proof of claim.
