Letter to Employee (Requesting Medical Information)
Summary
This precedent is a letter from an employer to its employee requesting medical information from the employee's doctor to assess the employee's condition and need for accommodation in the accommodation process. This precedent includes practical guidance and drafting notes. Where an employee has requested accommodation for a disability or other medical needs, the employer may require additional information about the employee’s limitations and needs from the employee’s physician or other health care provider. In this case, the employer may either request for the employee to provide any supporting documentation, or may request permission to contact the health care provider.
