Summary

This template is a durable power of attorney to appoint an agent to make decisions regarding the principal's health care in the event that the principal becomes incapacitated and unable to make the decisions for himself or herself. This template includes practical guidance, drafting notes, and alternate and optional clauses. A living will is a separate document that supplements a durable power of attorney for health care decisions and provides guidance to the agent(s) for health care decisions.
