This template is an Estate Planning Questionnaire that aids in compiling information necessary to plan a Florida estate. An attorney can complete this template at the client interview, after a point-by-point discussion of the client's estate-planning desires and options. This template includes practical guidance and drafting notes. This template incorporates Florida law and is intentionally brief and general; it should be used as an outline of topics to discuss with the client at the initial meeting. The worksheet is designed for efficient compilation of necessary data and as a source of reference in formulating the client's estate plan and in drafting any instruments necessary to effect that plan. This worksheet should be used to supplement the basic personal information (see Facts to Gather for Estate Planning Checklist (FL)) and Financial Information Questionnaire (Long Form) (FL) previously obtained from the client. Also use in conjunction with the Trust Drafting Worksheet (FL) (if the estate plan involves one or more trusts), and the Estate Tax Worksheet (FL) (if calculation of the gross estate is needed for the client and the client's spouse.
