This template is a disability accommodations policy for use by Oregon based employers. This template includes practical guidance and drafting notes. This template is intended for private employers with six or more employees. It contains an acknowledgment, and includes information about notice requirements, and other obligations of an employer to accommodate disabilities in the workplace. Employers may include this policy in an employee handbook or distribute it as a standalone policy. For a non-jurisdictional employee handbook, see Employee Handbook. Its language has been customized to comply with Oregon and federal law.
