This template is a direct deposit authorization form for use by Florida employers for purposes of making payroll and other payments to employees by way of electronic deposit to the bank of the employee's choosing. This template includes practical guidance and drafting notes. This template is a basic authorization form and is intended for private employers. Its language has been customized to comply with Florida and federal law. As a result, the text of this template does not differ from the non-jurisdictional Direct Deposit Authorization Template, except that the drafting notes address issues under both Florida and federal law.
