This form is a complaint procedure for use by an employee who feels that he or she has been harassed, discriminated, or retaliated against, or who witnessed any such conduct by an employee, contract worker, customer, vendor, or anyone who does business with the company. This template includes practical guidance and drafting notes. This form is intended for private employers.
