This template questionnaire is intended for use by a compensation committee to assist it with its performance assessment. This template includes practical guidance and drafting notes. This questionnaire should be provided to compensation committee members and others who interact with the compensation committee to receive additional feedback and assessment. This may include human resources (HR) executives, compensation consultants and independent legal advisers.
