Community Health Needs Assessment (CHNA) Checklist
Summary
This checklist assists nonprofit hospitals and health systems in conducting a community health needs assessment (CHNA). A CHNA is a systematic process used by nonprofit hospitals and other health organizations to identify and analyze the health needs of a specific population within a defined community. The Patient Protection and Affordable Care Act (ACA), Pub. L. No. 111-148, requires that nonprofit hospitals and health systems conduct a CHNA every three years and adopt an implementation strategy to meet the needs the CHNA identifies. I.R.C. § 501(r)(3).
