This template is a client intake template that is meant for use with potential clients who come in for consultations about a federal case. You may provide the potential client with this template to fill out before you conduct the consultation. This template includes practical guidance, drafting notes and an optional clause. The intake template should ask for all the key information about the contemplated matter, including a description of the matter, the proposed client's contact information, and any applicable deadlines. You should hold off on accepting representation until you have reviewed (and possibly investigated) the information in the completed template, including performing an in-depth conflicts check. This will protect you from having to withdraw from representation later on, for example, if you discover a conflict.
