Summary

This template is an affidavit of service which must be filed within 20 days after service of the notice of filing of a mechanic's lien claim in Pennsylvania and must set forth the date of filing and manner of service of such filing. This template includes practical guidance and drafting notes. Failure to file the affidavit within the specified time frame is grounds for striking the claim.
