Summary

This affidavit of publication template may be used to provide an affidavit relating to the publication of the notice of formation of a New York professional service limited liability company (PLLC). This template includes practical guidance and drafting notes. A New York PLLC must, within 120 days after the effective date its formation, publish a copy of its articles of organization or a compliant notice in newspapers in compliance with statutory requirements. N.Y. Ltd. Liab. Co. Law § 1203(c)(2)(A). Upon completion of the publication of notice, but before the expiration of the 120-day period, the PLLC must file with the New York Department of State proof of publication, consisting of a certificate of publication, along with affidavits from the relevant newspapers confirming publication.
