Summary
This Affidavit is a standard form that attorneys can use in a New York state court litigation to provide sworn testimony for a variety of purposes. It includes a caption, introductory language, a signature line, and a notary provision. It also includes practical guidance, drafting notes, optional clauses, and alternate clauses. The body of an affidavit varies greatly depending on its purpose. This template provides sample language that can be modified for a variety of uses. An affidavit is a sworn statement that you can use in a multitude of legal situations. For example, you can use affidavits to: • Put evidence before the court • Attest to facts at issue in support of motions • Authenticate documents • Confirm service was effected An affiant must sign the affidavit before a notary public or other authorized official.
