Addendum to Proof of Claim


This form is an addendum to a proof of claim that is filed by a creditor in a bankruptcy proceeding. This template includes practical guidance and drafting notes. The addendum should set forth the pertinent background facts of the relationship between the debtor(s) and the creditor, and the manner in which the creditor believes that the claim arose. While it is not necessary to prove the legal elements of specific types of claims in the addendum, the addendum should set forth the relevant factors.
