This template is a living will declaration that may be used in Ohio to direct medical treatment if an individual has a terminal condition or is in a permanently unconscious state. This template contains practical guidance, drafting notes, and optional clauses. Under Ohio law, a living will declaration is applicable only to individuals in a terminal condition or a permanently unconscious state. A health care power of attorney is used to direct medical treatment in other circumstances. If a person chooses to execute both documents, then the documents should be provided to the person's health care facility and physician.
