COBRA Early Termination Notice


Summary

This template is a COBRA early termination notice used to alert individuals that their COBRA continuation coverage under an employer-provided group health plan is ending before the end of the maximum coverage period. The notice includes practical guidance, drafting notes, and optional clauses. The plan or COBRA administrator is required to provide this notice to any affected qualified beneficiary as soon as possible following the determination that continuation coverage is to terminate. 29 C.F.R. § 2590.606-4(d). The notice must state the reason(s) for the early termination, the date coverage terminates, and any conversion rights the individual may have to elect alternative group or individual coverage. The administrator must furnish the notice in a manner that meets the general ERISA disclosure requirements of 29 C.F.R. § 2520.104b-1 (which includes rules for delivery by electronic media).
